1 – Your Business Name Doesn’t Match CP-575

To complete your PECOS Medicare application you must upload a copy of your CP-575 letter along with other documents.

The CP-575 is a letter you would have received from the IRS on which you will find your official business name, your tax identification number (employer identification number) and other business information.

You must ensure that your business name exactly on your business check and NPI registry EXACTLY matches your CP-575.

2 – You Don’t Reply To Email Requests

I have completed over 78 initial enrollment this calendar year and every time a Medicare MAC required additional information I received an email and replied within 24-hours.

You will often haev 30-days to reply to emails requesting information and corrections, and the requested information is always easy to gather, but if you don’t reply within the designated timeframe your application will be rejected and you will be required to start over again.

3 – Failed Site Visit

The third most common mistake in PECOS really doesn’t happen in PECOS. If you have a practice location and you are a physical therapist enrolling as such, CMS requires a site visit. 

Your business must be open and operating. You must have visible signage and hours of operation posted. Other required criteria to pass inspection may vary by MAC requirements but most therapists who fail the site visit fail because they are not open for business yet, the do not have signage available, or they do not have hours of operation clearly posted.
