Step 1 – Go to: https://www.anthem.com/provider/enrollment/
Step 2 – Select your state from the 14 options listed in the drop-down menu.
Step 3 – Choose your provider type from the second drop-down menu. Physical therapists, occupational therapists, and speech-language pathologists should choose “Ancillary Provider or Allied Health Provider.”
Step 4 – “Does your organization have an existing Availity account?”

Step 5 – After clicking the Submit button you will be taken to this PDF
where you will be prompted to complete the Provider Maintenance Form – Provider Application / Add Provider Form. You will also be asked to grant Anthem BCBS access to your CAQH application.
